There is a method to the madness:
No matter the maddness, no matter the reason, there is a method for it. No matter how mad things seem, there is a method that is used. A pattern in other words. When someone does something deemed Mad, they will often have a very specific way of doing it. They will always return to that same idea, that same way of dealing with things.
So in reality Madness is not something that is unplanned or unthought of, its really something that shows thought and pattern. No matter what or who, no matter the situation, the method is real and constant.
Look at the serial killers in our history, Bundy and the rest. Yes it was madness, but there was a method that came back time and time again. Unique to each it was something that they always returned to.
So those who say that maddness are random acts....look at what history has to show....

Jeff violently pushed the double-doors open with one hand, his sawed-off shotgun in the other.
"EVERYBODY, LISTEN THE FUCK UP," he yelled, "THIS IS A ROBBERY!"
He unsuccessfully attempted to mask his voice. He felt the cold, air-conditioned air cooling the skin through the eye-holes of the black ski-mask he was wearing.
As he walked towards the desk, he turned his upper body sideways, still walking forward, and pulled the trigger of his Remington. The other people in the bank watched as the successful twenty-six year old businessman's head detatched from his body and broke apart, spewing brain matter and blood all over the wall.
Turning back towards the front, he unzipped his parka with his free hand and pulled out a large, black duffel bag. As he lifted his arm to set the bag on the counter, he shook the bag open. The teller, a thirty-something black woman, reached out to grab it.
"I WANT AS MUCH MONEY AS YOU CAN FIT IN HERE, PRONTO!" His head bobbed up and down as he yelled.
The teller scurried back into the other room, duffel bag in hand.
Jeff set his weapon on the counter and started to climb over, careful not to let his hand off of it.
*BOOOM!*
The counter shredded as the BB's dug into it, chipping pieces off. A cloud of smoke puffed above the counter as more of the BB's hit a monitor sitting behind the counter.
*CHI-CHICK!*
Seconds later, the black woman returned from the back room, full duffel bag in hand. As she set it on the counter, the robber demanded that she open it up. She got visibly scared. Slowly, she reached towards the zipper. The robber's eyes watched as she tugged very lightly at the zipper.
*zzzzziiiiiiiipppppp*
*BOOOM! CHI-CHICK!*
The lady's upper torso separated from her stomach and blew into chunks. Blood splattered violently on the wall ten feet behind her. The area of the mask around his mouth curled up, showing that he was obviously smiling.
As he zipped the bag back up, he pulled it off of the counter. He was very visibly burdened from the weight of $2.5 million.
*BOOOM! CHI-CHICK!*
The glass shattered as each of the BB's blew through it. Jeff walked up to the door, stepped through the area where the glass was before, and--
*THUMP!*
"UMPH!" Was the sound he made as two burly FBI agents grabbed the gun out of his hand and threw him to the ground. Wriggling on the ground with two large men on him, restraining him only seemed to anger him more.
*AAARRGHHGHHG," he screamed as he threw both men off of him, and he reached down to his shin. He quickly pulled his pants leg up, revealing a large hunting knife, tight in its sheath. He grabbed the handle and yanked upwards, wincing slightly as the blade dug into his skin, slicing.
By the time he was fully stable and standing, the agents already had their weapons drawn and ready to fire.
"Put the weapon down, Mister Anderson! We can end this peacefully!"
The would-be robber reached up with his free hand and slowly pulled the mask off of his head.
He smirked as he reached up with his other arm, knife glistening. He drew the knife closer to his throat.
"There's a method to the madness, boys. If you fuck up one thing, don't fuck up the next."
